For November and December 2024, call 207-941-0080 for any last medical concerns or refills. The covering provider may not fill controlled substances and may not do referrals. And this phone number may be out of service by December 31, 2024. If you have an emergency, call 911 or go to the nearest emergency room.

We will get to them on a first come first served basis, allow at least 20-30 days. There may be a charge for scanning and mailing records. The current allowed charge is $5 for the first page and 45 cents for each additional page. Your new provider may not want the entire chart(sometimes many hundreds of pages), maybe just the last year or two, to keep the volume down. Ask them what they want. A pdf records release form can be downloaded from this website.
After December 30 2024, all records requests must be mailed to the Brewer address. Records will be stored at a secure facility for years.
